Talphera (TLPH): Dialysis Circuit Anticoagulant Approaches a Binary Readout

Talphera is no longer the opioid-delivery company that once sold DSUVIA. The San Mateo specialty firm now exists as a single-asset bet on Niyad, a lyophilized form of nafamostat studied as a regional anticoagulant for the extracorporeal circuit in continuous renal replacement therapy. Midyear enrollment in the registrational NEPHRO CRRT study reached three-quarters of the planned cohort, and management is turning from site activation toward a commercial story that still has no product revenue. The investment case is whether a small, placebo-controlled device trial plus a guideline mention can convert a cash-constrained microcap into an approved hospital product before the balance sheet forces another raise.

The cash stack at midyear was $17 million. Combined research and selling costs stayed near $4 million for the June quarter. Those figures look tidy until they sit next to the going-concern paragraph that says current resources do not cover the next twelve months of planned operations without more capital. Conditional financing of roughly $16 million remains gated on study success and a minimum trading price. CorMedix holds a sixty-day exclusive window to negotiate an acquisition after topline results, which is both a potential exit and a ceiling on competing bids.

The next few months resolve a simple question. Does the blinded study produce a clean separation on post-filter clotting time, unlocking the remaining capital and the CorMedix conversation, or does a miss leave a pre-revenue issuer with an accumulated deficit near half a billion and a warrant book larger than the listed float? Shares recently traded near $1.16 against a roughly $60 million headline capitalization. That price already embeds a non-trivial chance of a positive readout. The open issue is whether the market is paying for an approved hospital franchise or merely for the option that CorMedix may choose to pursue.
