Mesoblast has crossed from a two-decade development story into a genuine commercial company, and the investment debate is whether the first full year of Ryoncil sales funds the much larger adult and chronic-pain pipeline or merely proves a high-priced pediatric niche. The product is the first mesenchymal stromal cell therapy cleared by the Food and Drug Administration, sold into pediatric steroid-refractory acute graft-versus-host disease after a late-March launch in the prior fiscal year. Net product sales reached $115 million in the year ended late June. That print converts a previously royalty-only P&L into a specialty-drug franchise, but it still sits inside a company that lost money and continues to spend on three late-stage programs.
The commercial engine is real and concentrated. More than fifty United States transplant centers now stock the product, coverage reaches nearly all insured lives after a J-code arrived last October, and fourth-quarter net sales of $36 million show the launch is still adding volume rather than plateauing. Gross profit excluding amortization was $110 million, which is enough on a standalone product basis to fund a lean specialty sales force. The tension is that research spending nearly tripled once a prior-year inventory credit is stripped out, so the cash generated by Ryoncil is being recycled into adult graft-versus-host disease, chronic low back pain, and heart-failure programs that do not produce revenue yet.
Cash at year-end was $103 million after a related-party refinance that retired higher-cost Oaktree and NovaQuest paper. Operating cash use fell to $44 million for the full year and to $13 million in the second half, which is the first evidence that collections can shrink the burn. The market capitalizes the equity near $2 billion, a multiple that already prices adult label expansion and a back-pain readout rather than pediatric peak sales alone. The next twelve months resolve whether pediatric volume keeps compounding fast enough to carry that pipeline without another equity raise.
