IRIDEX (IRIX): Glaucoma Probes Carry a Fragile Turnaround

IRIDEX is a small ophthalmic-laser maker whose second-quarter print asks a single question: whether rising use of disposable glaucoma probes can carry a still-thin cash balance through a headquarters move that itself disrupts the larger retina franchise. The company sold more Cyclo G6 probes than in any recent quarter, and operating cash turned positive even as reported sales declined. That combination is the entire bull case in miniature. The market still prices the common as a distressed residual, and the tape is not obviously wrong.

The load-bearing event is not the headline sales miss. It is the probe print itself, and the mechanism underneath it. Cyclo G6 family sales rose to $3.9 million as physicians used more single-use delivery devices on an already-installed console base. Probe unit volume climbed 35% from the year-ago quarter, which is the kind of consumable lift that, if organic, converts an installed-base story into recurring cash. Management attributes the lift to MedScout targeting of under-used accounts, Medicare local coverage determinations that widened the clinical conversation around MicroPulse therapy, and higher United States selling prices. The same call also conceded that distributors in Japan and Europe pulled product forward to cover registration blackouts tied to the San Jose move. Back out that stocking and the residual growth still sits above 15%, which is useful, but it is no longer a clean utilization read.

The tension is cash, mix, and listing, not product science. Cash ended July at $4.7 million after a $0.1 million sequential rise. That was a genuine operating achievement on a still-unprofitable quarterly sales base. That cushion sits against purchase commitments due inside a year that dwarf cash, a convertible note still on the books, and Series B preferred with a $6.0 million liquidation preference sitting above common. Nasdaq sent a minimum-bid notice in late August after the shares spent thirty sessions under a dollar, opening a cure window measured in months rather than weeks. Retina sales, still the larger product pool, fell to $6.5 million on Middle East disruption, China destocking, and delayed European registration for the PASCAL platform. Gross margin barely held in the mid-thirties, so the higher-margin probe mix did not yet pay for the capital-equipment hole.

The next two prints resolve whether the probe spike was a new run-rate or a pre-move inventory wave, and whether retina can post even low-single-digit second-half growth without Middle East volume. Full-year sales guidance of $51 million to $53 million already excludes that region and implies only modest pro forma growth. The San Jose relocation and the contract-manufacturing shift are the operational levers that either restore margin in 2027 or consume the remaining cash first. Shares last changed hands near $0.61, for a market value of about $10.7 million, which is a liquidation-adjacent multiple on a company that just printed its first cash-flow-positive quarter of the year.
