Guardant Health occupies the intersection of two inflection points: the FDA approval of Guardant360 Liquid CDx and UnitedHealth Group's decision to cover the Shield blood test. The company is a precision oncology company that has spent over a decade building the Smart Platform, a multi-modal blood and tissue testing engine that now spans therapy selection, recurrence monitoring, and early cancer screening. The strategic question is not whether the technology works, but whether the commercial ramp and reimbursement environment can support the capital intensity of the model before the TwinStrand patent judgment becomes a meaningful drag on cash flow.

The most important recent development is the FDA approval of Guardant360 Liquid CDx, which integrates genomic and epigenomic insights into a single liquid biopsy panel and is the only FDA-approved liquid biopsy test combining both modalities. The approval consolidates multiple legacy offerings into one product, simplifies the clinical ordering pathway, and positions Guardant to capture a larger share of the therapy selection market as ADLT designation is expected in the first half of 2027. The central tension is the TwinStrand Biosciences and University of Washington patent judgment, which awarded over $245 million in damages, accrued royalties, and interest as of the final judgment entered in August 2026. The court ordered an ongoing royalty of 6 percent on a defined set of covered products and services through patent expiration in March 2033. The covered products represent approximately 90 percent of Guardant's total revenue during the relevant period. The company has appealed to the Federal Circuit, and both patents have been preliminarily invalidated in ongoing USPTO proceedings, but the financial overhang remains a material uncertainty for the next two to three years.

The catalyst is the H1 2027 ADLT designation for Guardant360 Liquid CDx, which would trigger the full commercial rollout and a step-up in Medicare reimbursement. The combination of that event, the UnitedHealth Shield coverage, and the SOLAR post-approval study enrollment provides a dense catalyst calendar.
