Quest Diagnostics (DGX): The Volume Supercycle Hides a Mix Problem

Quest Diagnostics stands at the intersection of two forces that should pull the stock in opposite directions: a genuine volume acceleration in clinical testing and a business-mix shift that is quietly diluting revenue per requisition. The second quarter revenue print, up 10.2% year over year, looks clean on the surface, but the strong growth in requisition volume came alongside a 2.8% decline in revenue per requisition, a divergence that only the Corewell Health and Fresenius Medical Care relationships explain.

The strategic tension is whether the volume engine can outrun the mix dilution. Excluding the two new partnerships, organic volume growth was roughly 4% and revenue per requisition actually rose about 3% as test counts per requisition climbed. The adjusted EPS, up roughly 19% year over year, reflects both the volume surge and the Invigorate cost program delivering its third year of structural savings, but it also benefited from a $11 million favorable tax resolution that does not repeat. The $500 million senior notes refinancing completed in June and the new $1 billion buyback authorization signal management confidence, yet the net debt to adjusted EBITDA multiple remains elevated relative to the growth trajectory.
